Current PPO Deductible Plan

This health plan supplies services at a higher level of benefits when members use contracted health care providers. The PPO also provides coverage for services rendered by health care providers who are not part of the PPO network; however, the plan member generally shares a greater portion of the cost for such services.

The deductible is the amount you pay for most covered services before your health plan starts to pay. When you go to a provider that is in the plan's network, before you meet the deductible you may pay a discounted amount that has been negotiated with the provider. The deductible resets at the beginning of the calendar year or when you enroll in a new plan.

PPO Copay Plan

This PPO health plan includes a copay (fixed dollar amount) for in-network office visits, specialist visits, emergency services and urgent care. This health plan also includes tiered copays for generic, formulary and non-formulary prescription drugs.

The convenience of copays for your visits and prescriptions includes higher premiums.

High-Deductible Plan with HSA

H&P offers you a high-deductible health plan, also known as an HSA-eligible plan, as an option. This is a consumer-driven health plan that includes a higher deductible which is then offset with lower monthly premiums.

This plan includes two tax-deferred spending account options.

  • It is also paired with a Health Savings Account (HSA) to allow users to contribute tax-free contributions into a savings account to be used for qualified health-related expenses. To help you cover the cost of your medical expenses, H&P will deposit a prorated annual contribution of $500 into your HSA. You must open your HSA to contribute into the plan. If you do not open your HSA, you will forfeit the employer contribution.
  • A Limited Purpose Flexible Spending Account (LPFSA) is a special type of flexible spending account (FSA) that is available to consumers enrolled in a health savings account (HSA). Consumers can use LPFSAs to pay for vision and dental expenses before they've met their insurance deductible.
